Advanced Healthcare Directive/Living Will - A Living Will is a legal document that specifies to healthcare providers and others the type of treatment that you consent to should you become too ill or incapacitated to make decisions yourself.
Durable Power of Attorney - A Durable Power of Attorney allows you to authorize someone else to handle many types of transactions on your behalf like banking, bills, tax returns, insurance benefits, government benefits, and more. It remains in effect if you become incapacitated, due to an illness or an accident.
Medical Power of Attorney - A Medical Power of Attorney is different from a Durable Power of Attorney. A Medical POA appoints someone to make healthcare decisions for you should you become incapacitated or unable to act for yourself. It is often used in conjunction with a Living Will.
Medical Records Release - Without a Medical Records Release, no other person can access your medical records. This Release allows specific individuals to receive copies of your medical records or health information.
